Mechanism of Action

The exact mechanism of Silvec 1% is not fully elucidated, but its antimicrobial activity involves:

• Silver moiety: The silver ions bind to bacterial cell membranes, causing structural changes, including distortion and weakening of the cell wall, reducing bacterial viability. After penetrating the cell, silver may bind to DNA, preventing bacterial replication. The ratio of silver to bacterial DNA is sufficient to inhibit bacterial growth while sparing mammalian epithelial cells, allowing tissue regeneration.

• Sulfadiazine moiety: Provides bacteriostatic action, interfering with bacterial growth in sensitive organisms.

After topical application, a small amount of sulfadiazine may be absorbed systemically; in adults, up to 10% may be absorbed, with 60–85% excreted in the urine. In children with burns covering 13% of body surface area, urinary sulfadiazine levels reached 31.8 mg/L.

Indications

• Topical prophylaxis against bacterial colonization and infection in burn wounds

• Topical antibacterial management of contaminated or infection-prone wounds other than burns

Dosage and Administration

• Cleanse burn or affected wound areas before application.

• Apply Silvec 1% Cream once or twice daily to a thickness of approximately 1/16 inch (1.5 mm).

• Ensure that the wound area remains covered at all times.

• Reapply the cream after hydrotherapy or if removed by patient activity.

• Dressings may be used if required, but reapplication must follow immediately after dressing removal.

• Continue treatment until satisfactory healing occurs or the wound is ready for grafting. Do not withdraw therapy prematurely while infection risk persists.

Contraindications

• Hypersensitivity to silver, sulfadiazine, or any component of the cream

• Pregnant women approaching or at term

• Premature infants and newborns under 2 months of age

Precautions and Warnings

• Monitor renal and hepatic function, as impaired elimination may lead to accumulation.

• Avoid combination with enzymatic debriding agents, which may be inactivated by silver.

• Monitor serum sulfadiazine levels in patients with extensive burns.

• Regularly check urine for sulfa crystals in patients on prolonged therapy.

• Use in pregnancy only if clearly justified (Category B).

Drug Interactions

• Enzymatic debriding agents: May be inactivated by silver

• Oral hypoglycemics and phenytoin: Action may be potentiated in patients with large burns; monitor levels

• Cimetidine: May increase the risk of leukopenia in extensive burns
