Amlodipine Besilate inhibits the influx of calcium ions into vascular smooth muscle and myocardial cells. This results in vasodilation, reduced peripheral vascular resistance, and a gradual lowering of blood pressure. Its long duration of action ensures smooth blood pressure control without significant reflex tachycardia.
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in vascular smooth muscle and adrenal glands. By preventing angiotensin II–mediated vasoconstriction and aldosterone release, telmisartan promotes vasodilation and reduces sodium and water retention.
The combination of amlodipine and telmisartan provides an additive antihypertensive effect, achieving better blood pressure control than either agent alone.

Telmacal Tablet 10 mg + 40 mg is indicated for:
Treatment of essential hypertension in adults
Patients whose blood pressure is not adequately controlled with amlodipine or telmisartan alone
Patients requiring combination therapy for optimal blood pressure management
Dosage should be individualized based on patient response and clinical condition
The usual recommended dose is one tablet once daily
Tablets may be taken with or without food
Blood pressure should be monitored regularly during therapy
Dose adjustment may be required in elderly patients or those with hepatic impairment
Hypersensitivity to amlodipine, telmisartan, or any component of the formulation
Pregnancy and lactation
Severe hepatic impairment or biliary obstructive disorders
Hypotension may occur, particularly in volume-depleted patients
Peripheral edema may occur due to amlodipine
Use with caution in patients with renal or hepatic impairment
Avoid concomitant use with other agents acting on the renin-angiotensin system unless clearly indicated
Commonly reported side effects include:
Dizziness or headache
Peripheral edema
Flushing
Fatigue
Hypotension
Most adverse effects are mild to moderate and usually resolve with continued treatment or dose adjustment.
Other antihypertensive agents may enhance blood pressure-lowering effects
NSAIDs may reduce antihypertensive efficacy and impair renal function
Potassium supplements or potassium-sparing diuretics should be used with caution
Telmacal Tablet is contraindicated during pregnancy due to the risk of fetal harm
Breastfeeding is not recommended during therapy
Store below 30°C in a cool and dry place
