Premil 2 mg Tablet is an oral antidiabetic medication used in the management of type 2 diabetes mellitus (non-insulin-dependent diabetes mellitus). It is prescribed as an adjunct to diet and exercise for patients whose blood glucose levels cannot be adequately controlled through lifestyle modification alone. Premil 2 mg may also be used in combination with metformin to achieve better glycemic control when monotherapy is insufficient.
Premil belongs to the meglitinide analogues class of antidiabetic drugs. This class is particularly effective in controlling postprandial blood glucose levels due to its rapid onset and short duration of action. Premil 2 mg is designed to be taken before meals, allowing flexible dosing that matches individual eating patterns and reducing the risk of prolonged hypoglycemia.

Premil 2 mg is indicated for:
Type 2 diabetes mellitus in adults as an adjunct to diet and exercise
Use as monotherapy in patients whose hyperglycemia is not adequately controlled by diet and exercise alone
Combination therapy with metformin in patients whose blood glucose levels are not controlled with either Premil 2 mg or metformin alone
Meglitinide Analogues
Premil 2 mg lowers blood glucose by stimulating insulin release from pancreatic beta cells. It works by inhibiting potassium efflux through ATP-regulated potassium channels in the beta-cell membrane. This action leads to membrane depolarization, opening of voltage-dependent calcium channels, increased calcium influx, and subsequent insulin secretion. The insulin release is glucose-dependent, making Premil especially effective in controlling meal-related glucose spikes.
Premil 2 mg should be taken immediately before or up to 30 minutes before meals.
The drug may be taken two, three, or four times daily depending on the number of meals
If a meal is skipped, the corresponding dose should also be skipped
If an extra meal is taken, an extra dose should be taken with that meal
Missed doses should not be taken between meals
Dosage guidelines:
HbA1c <8%: Starting dose 0.5 mg before each meal
HbA1c >8%: Starting dose 1–2 mg before each meal
Dose may be titrated at 1–2 week intervals
Maximum dose: 4 mg before each meal, up to 16 mg daily
CYP450 inhibitors such as azole antifungals and macrolides may increase Premil levels
CYP450 inducers like rifampicin, carbamazepine, and phenobarbital may reduce its effectiveness
Highly protein-bound drugs such as NSAIDs may increase hypoglycemia risk
Co-administration with other hypoglycemic agents or drugs like salicylates, sulfonamides, MAO inhibitors, and beta-blockers may increase hypoglycemia risk
Premil 2 mg is contraindicated in patients with:
Type 1 diabetes mellitus
Diabetic ketoacidosis, with or without coma
Known hypersensitivity to the drug or its components
The most common adverse effect is hypoglycemia. Other possible side effects include:
Upper respiratory tract infections
Diarrhea or constipation
Nausea and vomiting
Skin reactions such as rash and urticaria
Increased risk of hypoglycemia if taken without meals
Insulin should be substituted during acute illness, surgery, trauma, or severe infection
Regular monitoring of blood glucose is recommended
Safety during pregnancy has not been established. Premil 2 mg should be used only if clearly needed. It is not known whether the drug is excreted in human milk; caution is advised during breastfeeding.
