Epicure 25 mg is an adjunctive anti-epileptic medication indicated for the treatment of partial-onset seizures in patients aged 4 years and older. For intravenous use, Epicure 25 mg injection is approved only for adults (16 years and above) when oral administration is temporarily not feasible.

Mechanism of Action:
Epicure 25 mg exhibits a high and selective affinity for synaptic vesicle protein 2A (SV2A) in the brain, which may contribute to its anticonvulsant effects. By modulating SV2A, Epicure 25 mg influences synaptic GABA release, which plays a key role in epileptogenesis. Additionally, Epicure 25 mg inhibits sodium (Na⁺) channels, further contributing to its anti-epileptic activity.
Indications:
Partial-onset seizures in adults and pediatric patients aged 4 years and above
Intravenous administration for adults when oral therapy is temporarily not feasible
Dosage and Administration:
Adults (≥16 years): Start with 50 mg twice daily (100 mg/day). Adjust between 25 mg twice daily (50 mg/day) and 100 mg twice daily (200 mg/day) based on therapeutic response and tolerability.
Pediatric patients (4–16 years): Dosage is weight-based and administered orally twice daily.
Injection: Administer intravenously over 2–15 minutes. Inspect solution for particulate matter or discoloration before use. Single-dose administration only.
Drug Interactions:
Rifampin: May reduce Epicure 25 mg concentrations; dose increase may be necessary.
Carbamazepine: Monitor for increased metabolite exposure; adjust carbamazepine dose if needed.
Phenytoin: Monitor phenytoin levels due to potential increases.
Levetiracetam: No additional therapeutic benefit observed with combination.
Contraindications:
Hypersensitivity to Epicure 25 mg or its inactive ingredients
Side Effects:
Common adverse reactions (≥5% and more frequent than placebo) include somnolence/sedation, dizziness, fatigue, and nausea/vomiting.

Precautions and Warnings:
Monitor for suicidal behavior and ideation
Neurological reactions: Advise against driving until patient adapts to therapy
Psychiatric reactions: Monitor for irritability, depression, anxiety, or aggressive behavior
Hypersensitivity: Discontinue immediately if bronchospasm or angioedema occurs
Gradual withdrawal recommended to avoid seizure recurrence
Use in Special Populations:
Pregnancy: Monitored through AED pregnancy exposure registry; limited human data
Lactation: Unknown effects; animal studies show excretion in milk
Hepatic impairment: Dose adjustment recommended
Renal impairment: No dose adjustment required
