Diltiazem is a benzothiazepine calcium channel blocker that inhibits the influx of calcium ions through L-type calcium channels in cardiac and vascular smooth muscle. By reducing intracellular calcium levels, Diltiazem produces:
Vasodilation: Relaxes coronary and peripheral arteries, lowering systemic vascular resistance
Negative chronotropic effect: Slows heart rate by acting on the sinoatrial (SA) node
Negative inotropic effect: Reduces myocardial contractility, lowering oxygen demand
Diltiazem is well-absorbed orally, with peak plasma concentrations reached within 1–2 hours. It undergoes extensive hepatic metabolism and has a half-life of approximately 3–4.5 hours, making twice-daily dosing suitable in most cases.
Litizem Tablet 60 mg is indicated for:
Hypertension: As monotherapy or in combination with other antihypertensive agents to lower elevated blood pressure and reduce cardiovascular risk
Chronic stable angina pectoris: To prevent anginal attacks and improve exercise tolerance
Supraventricular tachyarrhythmias: To control heart rate in conditions such as atrial fibrillation or atrial flutter
Adults: 60 mg orally, 2–3 times daily, depending on clinical response
Dose may be adjusted by the physician based on blood pressure, heart rate, and tolerance
Tablets can be taken with or without food
Regular monitoring of blood pressure and heart rate is recommended during therapy
Hypersensitivity to Diltiazem or other calcium channel blockers
Severe hypotension (systolic BP < 90 mmHg)
Acute myocardial infarction with pulmonary congestion
Second- or third-degree AV block (without a pacemaker)
Severe heart failure or cardiogenic shock
Litizem Tablet 60 mg is generally well tolerated. Common side effects include:
