Calcium-channel blockers
Verapamil inhibits the entry of calcium ions into slow calcium channels and voltage-sensitive areas of vascular smooth muscle and myocardial cells during depolarization. This action results in relaxation of coronary and peripheral vascular smooth muscle, leading to coronary vasodilation and improved myocardial oxygen delivery. Verapamil also suppresses automaticity and slows atrioventricular (AV) node conduction, making it particularly effective in managing supraventricular arrhythmias. These combined effects reduce heart rate, myocardial oxygen demand, and systemic blood pressure.
Veracal Tablet 80 mg is indicated for the treatment of:
Essential hypertension
Angina pectoris and prevention of re-infarction
Supraventricular arrhythmias
Verapamil injection (where applicable) is additionally used in:
Paroxysmal supraventricular tachycardia
Atrial fibrillation with rapid ventricular response (except in WPW syndrome)
Atrial flutter with rapid conduction
Extrasystoles
Acute hypertension and acute coronary insufficiency
Prophylaxis and treatment of ectopic arrhythmias, particularly ventricular extrasystoles during halothane anesthesia or adrenaline use
The dosage of Veracal 80 mg should be individualized by careful titration and administered with food.
For essential hypertension, the usual initial dose is 180 mg once daily in the morning. If adequate control is not achieved, the dose may be adjusted to:
240 mg once daily
180 mg in the morning plus 180 mg in the evening
240 mg every 12 hours
For angina pectoris, the usual dose ranges from 80 mg to 120 mg three times daily.
For arrhythmias in digitalized patients, 240 mg to 360 mg daily in divided doses may be required, depending on severity.
All dosage adjustments should be made under medical supervision.

Verapamil may interact with several medications. Plasma levels may increase with CYP3A4 inhibitors such as erythromycin, ritonavir, and cimetidine, and decrease with inducers like rifampicin and phenobarbital. Verapamil may enhance the effects of beta-blockers, digoxin, antihypertensives, and diuretics, increasing the risk of bradycardia or hypotension. It may also increase plasma concentrations of statins, immunosuppressants, lithium, and other commonly used drugs.
Veracal Tablet 80 mg is contraindicated in patients with:
Severe left ventricular dysfunction
Hypotension or cardiogenic shock
Sick sinus syndrome (without a pacemaker)
Second- or third-degree AV block (without a pacemaker)
Atrial fibrillation or flutter with accessory pathways such as WPW syndrome
Known hypersensitivity to verapamil
Verapamil is generally well tolerated. Common side effects include constipation, nausea, dizziness, headache, hypotension, bradycardia, edema, fatigue, rash, and transient elevation of liver enzymes.
Verapamil should be used during pregnancy only if clearly necessary, as it may cause fetal hypoxia due to maternal hypotension. It is excreted in breast milk; therefore, breastfeeding should be discontinued during treatment.
