Neocard Tablet 30 mg contains Diltiazem Hydrochloride, a calcium channel blocker widely used in the management of cardiovascular disorders. It works by relaxing the smooth muscles of blood vessels and improving blood flow, thereby reducing the workload on the heart. Neocard is especially effective in controlling angina and certain types of cardiac arrhythmias, while also helping to lower blood pressure in hypertensive patients.
Diltiazem belongs to the benzothiazepine group of calcium channel blockers and has both cardiac and vascular effects. By inhibiting the influx of calcium ions through slow calcium channels in cardiac and smooth muscle cells, Neocard helps maintain a balanced heart rhythm and improves oxygen delivery to the heart muscle.

Neocard Tablet 30 mg is indicated for the treatment and management of:
Chronic stable angina pectoris
Vasospastic (Prinzmetal’s) angina
Hypertension (high blood pressure)
Supraventricular arrhythmias, including atrial fibrillation and atrial flutter (rate control)
It may be used alone or in combination with other antihypertensive or antianginal agents, depending on patient needs and physician recommendation.

Diltiazem Hydrochloride acts by inhibiting calcium ion entry into myocardial and vascular smooth muscle cells. Calcium plays a key role in muscle contraction; by limiting calcium influx, diltiazem reduces myocardial contractility, slows atrioventricular (AV) nodal conduction, and causes vasodilation of coronary and peripheral arteries.
These effects result in decreased heart rate, reduced myocardial oxygen demand, improved coronary blood flow, and lowered blood pressure. Diltiazem is well absorbed after oral administration and undergoes extensive hepatic metabolism. Its balanced action on both the heart and blood vessels makes it particularly useful in patients who require control of angina along with blood pressure or heart rate management.
The dosage of Neocard Tablet 30 mg should be individualized based on the patient’s condition and response to therapy.
Adults: Commonly initiated at 30 mg taken 2–4 times daily, depending on clinical indication
Dose adjustments should be made gradually under medical supervision
Neocard should be taken orally with or without food, preferably at the same times each day to maintain consistent blood levels.

Neocard Tablet 30 mg is contraindicated in patients with:
Severe hypotension
Acute myocardial infarction with pulmonary congestion
Second- or third-degree AV block (unless a functioning pacemaker is present)
Sick sinus syndrome (without pacemaker)
Known hypersensitivity to diltiazem or any component of the formulation
Neocard is generally well tolerated. Possible side effects may include:
Headache
Dizziness or lightheadedness
Bradycardia
Peripheral edema
Flushing
Constipation
Most side effects are mild to moderate and usually resolve with continued therapy or dose adjustment.
Diltiazem may interact with beta-blockers, digoxin, and other antihypertensive agents, potentially enhancing their effects. Caution is advised when used with drugs metabolized by liver enzymes, as dose adjustments may be necessary.
The safety of Neocard during pregnancy has not been fully established. It should be used only if the potential benefit justifies the potential risk. Diltiazem is excreted in breast milk; therefore, caution is advised when administered to breastfeeding women.
