Traneta 5 mg improves glycaemic control by inhibiting the enzyme DPP-4, which is responsible for the breakdown of incretin hormones such as GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). Inhibition of DPP-4 increases active incretin levels, resulting in glucose-dependent stimulation of insulin secretion from pancreatic beta cells and suppression of glucagon release from alpha cells. This mechanism helps lower blood glucose levels without causing excessive hypoglycaemia.

Recommended dose: Traneta 5 mg once daily
May be taken with or without food, at any time of the day
When used with metformin, the metformin dose should be continued
When combined with a sulphonylurea, a lower sulphonylurea dose may be considered to reduce the risk of hypoglycaemia
Renal impairment: No dose adjustment is required
Dosage should be followed as prescribed by a registered physician.
Traneta 5 mg is a weak competitive and weak to moderate mechanism-based inhibitor of the CYP3A4 isoenzyme. Clinical studies have shown no clinically significant interactions with commonly used medicines, including:
Metformin
Glyburide
Simvastatin
Warfarin
Digoxin
Oral contraceptives
Clinically meaningful drug interactions are unlikely.
Traneta 5 mg is contraindicated in patients with:
Known hypersensitivity to the active substance or any excipients of the formulation
Possible side effects may include:
Hypoglycaemia (especially when used with metformin and sulphonylurea)
Nasopharyngitis
Cough
Pancreatitis (rare)
Most adverse effects are mild to moderate in severity.

Pregnancy Category B: Adequate and well-controlled studies in pregnant women are lacking
Traneta 5 mg should be used during pregnancy only if clearly needed
It is unknown whether the drug is excreted in human breast milk; caution is advised during lactation

Monitor patients for signs of pancreatitis
Dose adjustments of concomitant antidiabetic agents may be required to avoid hypoglycaemia
