Ticagrelor is a selective and reversible P2Y12 adenosine diphosphate (ADP) receptor antagonist. By binding to the platelet P2Y12 receptor, it inhibits ADP-mediated platelet activation and aggregation. Unlike thienopyridines, ticagrelor does not require metabolic activation and does not bind irreversibly to the receptor, resulting in a more rapid onset and offset of action.
Through effective inhibition of platelet aggregation, Tigel 90 mg reduces thrombus formation in patients with a high risk of ischemic cardiovascular events, thereby improving clinical outcomes following acute coronary syndromes.

Tigel Tablet 90 mg is indicated for:
Prevention of atherothrombotic events in adults with acute coronary syndromes
Patients with unstable angina, NSTEMI, or STEMI
Patients managed medically or undergoing PCI or CABG
Treatment should be initiated with a loading dose of 180 mg (two tablets of 90 mg)
Followed by 90 mg twice daily as maintenance therapy
Tigel 90 mg should be taken together with aspirin
Initial aspirin dose: usually 325 mg
Maintenance aspirin dose: 75–100 mg daily
Maintenance doses of aspirin above 100 mg should be avoided, as they reduce the effectiveness of ticagrelor
Tablets may be taken with or without food
Recommended duration of therapy is up to 12 months, unless clinically indicated otherwise
Avoid strong CYP3A4 inhibitors such as ketoconazole, clarithromycin, ritonavir, and nefazodone
Avoid strong CYP3A4 inducers including rifampicin, phenytoin, carbamazepine, and phenobarbital
Avoid simvastatin and lovastatin doses above 40 mg
Monitor digoxin levels when co-administered
Compatible with heparin, beta-blockers, ACE inhibitors, ARBs, and proton pump inhibitors
Hypersensitivity to ticagrelor or any excipients
Active pathological bleeding
History of intracranial hemorrhage
Moderate to severe hepatic impairment
Concomitant use with strong CYP3A4 inhibitors
Common side effects include:
Dyspnea
Bleeding
Headache
Dizziness
Nausea and diarrhea
Hypertension and hypotension
Pregnancy: Category C; not recommended unless clearly necessary
Lactation: Excreted in animal milk; breastfeeding should be discontinued or therapy avoided
Increased risk of bleeding
Avoid abrupt discontinuation due to increased risk of thrombotic events
Use with caution in hepatic impairment
Store below 25°C, protected from light and moisture. Keep out of reach of children.
