Angiotensin II is a key component of the renin-angiotensin-aldosterone (RAA) system and is responsible for vasoconstriction, aldosterone release, cardiac stimulation, and renal sodium reabsorption, all of which contribute to elevated blood pressure. Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor present in vascular smooth muscle and the adrenal gland.
By inhibiting these effects, Telma 40 mg promotes vasodilation, reduces aldosterone secretion, decreases peripheral vascular resistance, and lowers blood pressure. Telmisartan has a high affinity for the AT1 receptor, more than 3,000-fold greater than for the AT2 receptor. Since it does not inhibit angiotensin-converting enzyme (ACE), it does not interfere with bradykinin metabolism, reducing the likelihood of ACE-inhibitor-related cough. The antihypertensive effect is sustained over 24 hours with once-daily dosing.

Telma Tablet 40 mg is indicated for:
Treatment of essential hypertension in adults
Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease, including a history of coronary heart disease, stroke, or peripheral arterial disease
Cardiovascular risk reduction in adults with type 2 diabetes mellitus and documented target organ damage
The usual starting dose is 40 mg once daily
Blood pressure response is dose-related within the range of 20–80 mg
Most of the antihypertensive effect is seen within 2 weeks, with maximal effect by 4 weeks
Tablets may be taken with or without food
Dosage should be individualized based on patient response
No initial dose adjustment is required for elderly patients or those with renal impairment, including patients on hemodialysis, although close monitoring is advised
Concomitant use with NSAIDs may reduce antihypertensive effect and increase the risk of renal impairment
Use with aliskiren should be avoided in patients with diabetes
Caution is advised when used with other agents affecting the RAA system
Hypersensitivity to telmisartan or any component of the formulation
In hypertensive patients, commonly reported side effects include:
Sinus pain and congestion
Back pain
Diarrhea
In cardiovascular risk reduction patients, side effects may include:
Intermittent claudication
Skin ulcer
Most adverse effects are mild and transient.

Telma 40 mg is categorized as Pregnancy Category C during the first trimester and Category D during the second and third trimesters
It should be discontinued as soon as pregnancy is detected
Use during breastfeeding is not recommended due to potential risk to the nursing infant
Avoid fetal and neonatal exposure
Use with caution in patients with hepatic or renal impairment
Hypotension may occur, especially in volume-depleted patients
Avoid concomitant use of ACE inhibitors and ARBs
Store below 30°C
