Sliptin-M ER Tablet 50 mg + 1000 mg is a fixed-dose combination oral anti-diabetic medicine containing Sitagliptin 50 mg and Metformin Hydrochloride 1000 mg (Extended Release). It is used for the treatment of type 2 diabetes mellitus in adults whose blood sugar levels are not adequately controlled by diet, exercise, or single-drug therapy alone. The extended-release (ER) formulation of metformin allows for gradual drug release, ensuring sustained glucose control and better gastrointestinal tolerability.

Sitagliptin is a DPP-4 (dipeptidyl peptidase-4) inhibitor. It works by increasing the levels of incretin hormones such as GLP-1 and GIP. These hormones stimulate insulin secretion from the pancreas in a glucose-dependent manner and suppress excess glucagon release, thereby reducing glucose production in the liver. Sitagliptin primarily helps control postprandial (after-meal) blood glucose levels and has a low risk of causing hypoglycemia when used alone.
Metformin Hydrochloride (Extended Release) belongs to the biguanide class of anti-diabetic medicines. It lowers blood glucose levels by decreasing hepatic glucose production, reducing intestinal absorption of glucose, and improving peripheral insulin sensitivity. The ER formulation releases the drug slowly over time, helping maintain stable blood sugar levels throughout the day and reducing stomach-related side effects compared to immediate-release formulations.
Together, sitagliptin and metformin provide complementary mechanisms of action, resulting in improved overall glycemic control.
Sliptin-M ER 50 mg + 1000 mg is indicated for:
Adults with type 2 diabetes mellitus
Patients inadequately controlled with metformin alone
Patients already taking sitagliptin and metformin separately
Combination therapy when dual treatment is required
It is not suitable for patients with type 1 diabetes or diabetic ketoacidosis.
The dosage is individualized based on blood glucose levels, previous treatment, and kidney function. It is usually taken once daily with a meal, preferably the evening meal, to minimize gastrointestinal discomfort. The tablet must be swallowed whole and should not be crushed, split, or chewed, as this may damage the extended-release mechanism.
Common side effects include:
Nausea
Diarrhea
Abdominal discomfort
Headache
Upper respiratory tract infection
Serious but rare side effects include lactic acidosis (associated with metformin) and pancreatitis (associated with sitagliptin). Immediate medical attention is required if symptoms such as severe abdominal pain, rapid breathing, unusual weakness, or muscle pain occur.
