Antihypertensive Agent – Combined Alpha- and Beta-Adrenergic Blocker
Labetalol is a non-selective beta-adrenergic receptor antagonist with selective alpha-1 adrenergic blocking activity. Its beta-blocking effect decreases heart rate and myocardial contractility, while alpha-1 blockade leads to vasodilation, reducing peripheral vascular resistance and lowering blood pressure. Unlike pure beta-blockers, labetalol maintains cardiac output and cerebral blood flow, which is particularly important in patients with acute hypertensive crises.
Labetalol is rapidly absorbed after intravenous administration, and the onset of action is within 2 to 5 minutes. Peak effect is usually achieved within 5 to 15 minutes, making it suitable for emergency management. It is metabolized in the liver and excreted primarily via the kidneys. The elimination half-life ranges from 5 to 8 hours.

Management of severe hypertension and hypertensive emergencies
Perioperative hypertension
Hypertension in pregnancy, particularly in cases of pre-eclampsia or eclampsia, where rapid blood pressure control is necessary
Patients requiring intravenous therapy for acute blood pressure reduction
Adults:
Initial dose: 20 mg slow IV injection over 2 minutes
Additional doses of 20–80 mg may be administered at 10-minute intervals until the desired blood pressure is achieved
For continuous infusion: 1–2 mg/min, titrated according to response
Pregnancy:
Labetalol is commonly used in pregnancy for acute hypertension, but careful monitoring of maternal and fetal status is recommended
