Florium 5 mg Tablet is a calcium channel antagonist primarily used for the prophylaxis of migraine and the management of vestibular and neurological disorders. It is effective in preventing recurrent migraine attacks and is also used in conditions such as vertigo, motion sickness, peripheral vascular disease, and refractory epilepsy. Florium 5 mg is indicated for the following conditions:
Prophylaxis of classic migraine (with aura) and common migraine (without aura)
Symptomatic treatment of vestibular vertigo due to functional disorders of the vestibular system
Peripheral vascular disease (PVD)
Motion sickness
Refractory epilepsy resistant to conventional antiepileptic therapy

Florium 5 mg is a difluorinated derivative of cinnarizine and acts as a selective calcium channel antagonist. It prevents excessive transmembrane calcium influx, thereby protecting cells from calcium overload without disrupting normal cellular calcium homeostasis. Additionally, Florium 5 mg possesses mild antihistaminic and sedative effects. The drug is highly protein-bound, with approximately 99% binding to plasma proteins.
Migraine Prophylaxis:
Starting dose:
Patients under 65 years: 10 mg at night
Patients over 65 years: 5 mg daily
If depressive, extrapyramidal, or other unacceptable symptoms occur, treatment should be discontinued
If no significant improvement is observed after 2 months, the patient should be considered a non-responder
Maintenance Therapy:
Same daily dose administered for 5 days per week with 2 consecutive drug-free days
Maintenance therapy should be stopped after 6 months, even if effective, and restarted only upon relapse
Peripheral Vascular Disease:
10 mg twice daily
May be increased up to 30 mg per day if required
Vertigo and Motion Sickness:
Adults: 10–20 mg daily
Children over 40 kg: 5 mg daily
Epilepsy (Add-on Therapy):
Adults: 15–20 mg daily
Children: 5–10 mg daily
Galactorrhoea has been reported in some women taking oral contraceptives during the first two months of therapy
Hepatic enzyme inducers such as carbamazepine and phenytoin may increase the metabolism of Florium 5 mg, potentially requiring dose adjustment
Concomitant use with alcohol or CNS depressants may enhance sedative effects
Florium 5 mg is contraindicated in patients with:
Known hypersensitivity to the drug
History of depressive illness
Pre-existing Parkinson’s disease or other extrapyramidal disorders
Commonly reported side effects include:
Drowsiness or fatigue
Weight gain and increased appetite
Adverse effects reported with long-term use include:
Depression, particularly in patients with a history of depressive illness
Extrapyramidal symptoms such as bradykinesia, rigidity, tremor, akathisia, and orofacial dyskinesia
Less frequent effects include nausea, heartburn, gastralgia, insomnia, anxiety, galactorrhoea, dry mouth, muscle pain, and skin rash
The safety of Florium 5 mg during pregnancy and lactation has not been established. Use during these periods is not recommended unless clearly necessary.
Florium 5 mg may cause drowsiness, which can be aggravated by alcohol or other CNS depressants. Patients should avoid driving or operating machinery if affected. The drug is not intended for aborting acute migraine attacks, and dosage should not be increased during an attack. Long-term use may induce extrapyramidal or depressive symptoms and may unmask Parkinsonism, especially in elderly or predisposed patients. Careful monitoring is advised.
