Drugs for Erectile Dysfunction
Adafil 10 mg contains a selective phosphodiesterase type 5 (PDE5) inhibitor. PDE5 inhibition increases the concentration of cyclic guanosine monophosphate (cGMP) in smooth muscle cells. Elevated cGMP leads to relaxation of smooth muscles and increased blood flow into the corpus cavernosum, resulting in a penile erection during sexual stimulation.
PDE5 is also present in the smooth muscles of the prostate and bladder. Inhibition of PDE5 in these tissues increases cGMP levels, leading to smooth muscle relaxation in the prostate and bladder wall. This relaxation improves blood flow to the urinary tract and helps widen the bladder neck opening, thereby improving urinary symptoms associated with BPH.
Erectile Dysfunction:
Recommended starting dose: 10 mg
Dose may be increased to 20 mg or reduced to 5 mg based on patient response and tolerability
Maximum dosing frequency: once daily
Effective for up to 36 hours
Benign Prostatic Hyperplasia:
Recommended dose: 5 mg taken once daily at the same time each day
Combined Erectile Dysfunction and BPH:
Recommended dose: 5 mg taken once daily at the same time each day

Adafil 10 mg may interact with:
Nitrates such as nitroglycerin and isosorbide
Alpha-adrenergic blockers
Antihypertensive medicines
Alcohol
Antacids containing magnesium hydroxide or aluminum hydroxide
CYP3A4 inhibitors such as ketoconazole, itraconazole, erythromycin, ritonavir
HIV protease inhibitors
Rifampin, carbamazepine, phenytoin, phenobarbital
Grapefruit juice

Adafil 10 mg is contraindicated in:
Patients using nitrates, as this may cause severe hypotension
Patients with known hypersensitivity to Adafil 10 mg or any of its components
Common side effects include:
Headache
Dyspepsia
Back pain
Myalgia
Nasopharyngitis
Nasal congestion
Rare but serious side effects may include changes in color vision, sudden vision or hearing loss, Stevens-Johnson syndrome, exfoliative dermatitis, angina, stroke, myocardial infarction, severe hypotension, and tachycardia.

Adafil 10 mg is classified as Pregnancy Category B by the USFDA. It should be used during pregnancy only if the potential benefit justifies the possible risk to the fetus. There is no available data regarding its excretion in human breast milk, so caution is advised when administering to nursing women.
Caution is required in patients with:
Angina
Renal or hepatic impairment
Bleeding disorders
Concomitant use of nitrates, alpha blockers, alcohol, CYP3A4 inhibitors, or other PDE5 inhibitors
Medical supervision is recommended in these conditions.
