Acmecilin 250 mg is a broad-spectrum penicillin antibiotic used in the treatment of a wide range of bacterial infections caused by susceptible organisms. It works by inhibiting bacterial cell wall synthesis, leading to bacterial cell lysis and death. Acmecilin is effective against several gram-positive and gram-negative bacteria and is commonly used in respiratory, genitourinary, gastrointestinal, and systemic infections.

Acmecilin 250 mg is indicated for the treatment of infections caused by susceptible strains of the following organisms:
Genitourinary Tract Infections (including Gonorrhea):
Escherichia coli
Proteus mirabilis
Enterococci
Shigella species
Salmonella typhi and other Salmonella species
Non-penicillinase-producing Neisseria gonorrhoeae
Respiratory Tract Infections:
Non-penicillinase-producing Haemophilus influenzae
Staphylococci
Streptococci, including Streptococcus pneumoniae
Gastrointestinal Tract Infections:
Shigella species
Salmonella typhi and other Salmonella species
Escherichia coli
Proteus mirabilis
Enterococci
Other Infections:
Meningitis caused by Neisseria meningitidis
Bacteriological studies should be performed to identify causative organisms and determine their sensitivity to Acmecilin. Treatment may be initiated before susceptibility results are available.
Broad spectrum penicillins
Acmecilin 250 mg acts by binding to one or more penicillin-binding proteins located in the bacterial cell wall. This inhibits the final transpeptidation step of peptidoglycan synthesis, which is essential for cell wall integrity. As a result, bacterial cell wall formation is disrupted, leading to cell lysis through the action of autolytic enzymes.
Dosage depends on the type and severity of infection, route of administration, age, and body weight of the patient.
Oral: Commonly used for respiratory, gastrointestinal, urinary tract, biliary tract, and systemic infections.
Parenteral (IM/IV): Used for severe infections including meningitis and septicemia.
Special Routes: Intra-articular, intraperitoneal, and intrapleural administration may be used as a supplement to systemic therapy.
Adults typically receive higher doses, while children under 10 years are usually given half the adult dose unless otherwise specified.
Acmecilin 250 mg should be taken on an empty stomach, preferably 1 hour before or 2 hours after meals.
Acmecilin may reduce the effectiveness of oral contraceptives and oral typhoid vaccines. It can alter INR levels in patients on anticoagulants such as warfarin. Probenecid and sulfinpyrazone may increase Acmecilin levels by reducing its excretion. Concomitant use with allopurinol may increase the risk of skin reactions.
Acmecilin 250 mg is contraindicated in patients with known hypersensitivity to penicillins or any component of the formulation.

Common side effects include nausea, vomiting, diarrhea, and skin rashes. Rare but serious adverse effects may include severe allergic reactions, blood dyscrasias, liver enzyme elevation, nephritis, CNS toxicity, and Clostridium difficile-associated diarrhea.
Acmecilin is classified as Pregnancy Category B. It should be used during pregnancy and lactation only if clearly needed and under medical supervision.
Store at 20–25°C. Reconstituted oral suspension should be stored at 2–8°C and discarded after 14 days.
