Valsartil 40 mg is an oral antihypertensive medication belonging to the class of Angiotensin II Receptor Blockers (ARBs). It is primarily prescribed for the management of hypertension, reduction of hospitalizations in congestive heart failure (CHF), and to reduce mortality in patients who develop CHF after a myocardial infarction (heart attack).
The active ingredient, Valsartan, specifically blocks the AT1 subtype of angiotensin II receptors, preventing the narrowing of blood vessels and thereby reducing blood pressure. Valsartil 40 mg lowers blood pressure without significantly affecting the heart rate, making it suitable for patients with cardiovascular risk factors.

Angiotensin II Receptor Blocker (ARB)
Valsartil 40 mg is indicated for:
Hypertension: To lower high blood pressure and prevent complications such as stroke, heart attack, and kidney damage.
Congestive Heart Failure: To reduce hospitalizations in patients with CHF by decreasing strain on the heart.
Post-Myocardial Infarction: To reduce the risk of death and improve survival in patients who develop CHF after a heart attack.
Valsartil 40 mg is a selective angiotensin II receptor antagonist:
Mechanism of Action: Blocks angiotensin II from binding to AT1 receptors, preventing vasoconstriction.
Effect: Dilates blood vessels, reduces blood pressure, and decreases cardiac workload.
Selectivity: Over 20,000 times higher affinity for AT1 than AT2 receptors, minimizing off-target effects.
Metabolism: Minimally metabolized in the liver; primarily excreted in bile and feces.
Hypertension:
Typical dose: 80–160 mg once daily
Maximum dose: 320 mg daily
Maximum effect is usually achieved within 4 weeks
Heart Failure:
Initial dose: 40 mg twice daily
Can be increased up to 80–160 mg twice daily as tolerated
Post-Myocardial Infarction:
Initial dose: 20 mg twice daily
Titrated up to a target of 160 mg daily if tolerated
Administration Notes:
Should be taken on an empty stomach (food reduces absorption by ~40%)
No dose adjustment needed for elderly patients with mild to moderate renal or hepatic impairment
No clinically significant interactions with commonly used drugs such as digoxin, warfarin, furosemide, cimetidine, hydrochlorothiazide, amlodipine, atenolol, or glibenclamide
Caution: Concomitant use of potassium-sparing diuretics, potassium supplements, or salt substitutes may increase serum potassium levels
Known hypersensitivity to Valsartil or any component of the formulation
Severe hepatic impairment
Pregnancy (2nd and 3rd trimester)
Common adverse effects include:
Cardiovascular: Hypotension, dizziness, headache
Gastrointestinal: Nausea, abdominal pain, diarrhea
Other: Fatigue, cough, back pain, muscle cramps, rash, hyperkalemia
Rare but serious effects: Impaired renal function or allergic reactions
Pregnancy: Avoid during 2nd and 3rd trimester; discontinue immediately if pregnancy is detected
Lactation: Unknown if excreted in breast milk; weigh risk vs. benefit
Hepatic Impairment: Dose adjustment required; contraindicated in severe impairment
Renal Impairment: Monitor closely; dose reduction may be necessary
Heart Failure & Post-MI: Start with caution; monitor blood pressure and renal function
Likely manifestations: hypotension, tachycardia, or bradycardia
Management: Place patient in supine position, administer IV normal saline if necessary, and monitor vital signs
Store between 15–30°C
Protect from moisture and heat
Keep out of reach of children
Valsartil 40 mg selectively blocks angiotensin II from binding to AT1 receptors, preventing vasoconstriction, lowering blood pressure, and reducing cardiac workload. Unlike other antihypertensives, it does not significantly affect heart rate or other hormone pathways involved in cardiovascular regulation.
Pediatric Use: Safety and efficacy not established
Geriatric Use: No overall differences, but some elderly patients may be more sensitive
Hepatic Impairment: Mild to moderate: Max 80 mg once daily; severe: contraindicated.
