Trimela Cream 0.01% + 4% + 0.05% is a triple-combination topical formulation specially designed for the short-term treatment of moderate to severe facial melasma. It works by reducing hyperpigmentation, improving skin tone, and controlling inflammation associated with melasma. To achieve optimal results and minimize adverse effects, strict sun protection is essential during therapy.
Short-term treatment of moderate to severe melasma of the face
Patients should avoid sun exposure during treatment
Daily use of a broad-spectrum sunscreen with SPF 30 or higher is mandatory
Trimela Cream combines three proven dermatological agents:
Fluocinolone Acetonide (0.01%)
A topical corticosteroid with anti-inflammatory properties that helps reduce redness and irritation.
Hydroquinone (4%)
A depigmenting agent that inhibits melanin production by interfering with the tyrosine–tyrosinase pathway.
Tretinoin (0.05%)
A keratolytic agent that promotes skin cell turnover, enhances penetration of hydroquinone, and improves pigment dispersion.
Although the exact mechanism in melasma is not fully understood, the combined action effectively reduces hyperpigmentation.
Apply once daily at night, at least 30 minutes before bedtime
Cleanse face and neck gently, rinse, and pat dry
Apply a thin layer to affected areas, extending about ½ inch onto surrounding normal skin
Rub gently and evenly
Use sunscreen and protective clothing during the day
Moisturizers and cosmetics may be used during daytime
Pediatric Use:
Safety and efficacy in pediatric patients have not been established.
Avoid concurrent use of:
Medicated or abrasive soaps and cleansers
Cosmetics with strong drying effects
Products containing high alcohol concentrations or astringents
Other irritants or keratolytic agents
Medications with photosensitizing effects
Known hypersensitivity, allergy, or intolerance to any component of Trimela Cream
Mild to moderate redness, peeling, burning, dryness, or itching
Rarely, severe allergic reactions including breathing difficulty or asthma exacerbation
Pregnancy:
Pregnancy Category C. Trimela Cream contains tretinoin, a known teratogen. Animal studies suggest potential risks such as fetal death, growth alterations, and congenital malformations. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus.
Lactation:
It is unknown whether topical application results in sufficient systemic absorption to be excreted in breast milk. Caution is advised when administering to nursing mothers.

Hydroquinone and tretinoin may cause local skin irritation such as redness, peeling, burning, dryness, or itching
Discontinue use if severe irritation or hypersensitivity occurs
Fluocinolone Acetonide may cause reversible HPA axis suppression with prolonged or excessive use
Signs of systemic corticosteroid absorption include Cushing’s syndrome, hyperglycemia, and glucosuria
Discontinue treatment if HPA axis suppression is detected; recovery usually occurs after stopping therapy
