Indications

Tolmus 50 mg is indicated for the treatment of:

• Increased tone of skeletal muscles due to organic neurological disorders, including:

  • Injury of the pyramidal tract

  • Multiple sclerosis

  • Myelopathy

  • Encephalomyelitis

• Muscular spasm, muscular contracture, rigidity, and spinal automatism

• Vascular diseases such as:

  • Obliterative arteriosclerosis

  • Diabetic angiopathy

  • Obliterative thromboangiitis

  • Raynaud's disease

  • Diffuse scleroderma

• Selected cases of:

  • Post-thrombotic venous and lymphatic circulation disorders

Pharmacology

Tolperisone is a centrally acting muscle relaxant that works mainly at the reticular formation in the brain stem. It exerts its effect by blocking voltage-gated sodium and calcium channels, leading to reduced reflex activity in the spinal cord. This mechanism results in decreased muscle tone, reduced spasticity, and improved peripheral blood circulation.

Dosage & Administration

Adults:

• 150–450 mg daily, administered in 2 or 3 divided doses

• Maximum recommended dose: 450 mg per day

Children:

• 3 months to 6 years: 1–2 mg/kg/day in 3 divided doses

• 6 years to 14 years: 2–4 mg/kg/day in 3 divided doses

Usual Recommended Dose:

• Adults: 50–150 mg three times daily, adjusted according to patient response and tolerance

• Children (3 months to 6 years): 5 mg/kg/day in three divided doses

• Children (6 to 14 years): 2–4 mg/kg/day in three divided doses

Dosage should be followed strictly as prescribed by a registered physician.

Drug Interactions

Concomitant use of Tolmus 50 mg with methocarbamol may cause disturbances in visual accommodation. Caution is advised when used together with other centrally acting muscle relaxants.

Contraindications

Tolmus 50 mg is contraindicated in patients with:

• Known hypersensitivity to tolperisone or any component of the formulation

• Myasthenia gravis

• Nursing mothers

Side Effects

Possible side effects may include:

• Muscular weakness

• Headache

• Nausea

• Vomiting

• Rare hypersensitivity reactions such as itching, skin redness, and rash

Most side effects are mild and transient.

Overdose Effects

• Increased excitability has been reported following ingestion of high doses, especially in children

• In rare reported cases, fatal outcomes were associated with massive overdose
  Immediate medical attention is required in case of suspected overdose.
