Tetracef Injection 2 gm/vial is a bactericidal beta-lactam antibiotic that inhibits bacterial cell wall synthesis. It acts by binding to penicillin-binding proteins (PBPs), thereby blocking the final transpeptidation step required for the formation of the peptidoglycan layer of the bacterial cell wall. This disruption compromises cell wall integrity, leading to bacterial cell lysis and death. Cefepime exhibits strong activity against a wide range of organisms and maintains efficacy against many strains resistant to earlier-generation cephalosporins.

Tetracef Injection is indicated for the treatment of the following infections caused by susceptible microorganisms:
Moderate to severe pneumonia, including cases with concurrent bacteremia, caused by Streptococcus pneumoniae, Pseudomonas aeruginosa, Klebsiella pneumoniae, or Enterobacter species
Febrile neutropenia, for empiric monotherapy in selected patients (monotherapy may not be suitable for high-risk patients)
Uncomplicated and complicated urinary tract infections, including pyelonephritis, caused by Escherichia coli, Klebsiella pneumoniae, or Proteus mirabilis, including bacteremic cases
Uncomplicated skin and skin structure infections caused by methicillin-susceptible Staphylococcus aureus or Streptococcus pyogenes
Complicated intra-abdominal infections, when used in combination with metronidazole, caused by E. coli, viridans group streptococci, P. aeruginosa, K. pneumoniae, Enterobacter species, or Bacteroides fragilis
Tetracef Injection 2 gm/vial should be administered intravenously over approximately 30 minutes. The intramuscular route is reserved for selected mild to moderate infections.
Pneumonia: 1–2 g IV every 12 hours for 10 days
Febrile neutropenia: 2 g IV every 8 hours for up to 7 days or until neutropenia resolves
Mild to moderate UTIs: 0.5–1 g IV/IM every 12 hours for 7–10 days
Severe UTIs: 2 g IV every 12 hours for 10 days
Skin and soft tissue infections: 2 g IV every 12 hours for 10 days
Intra-abdominal infections: 2 g IV every 12 hours for 7–10 days with metronidazole
Dose adjustment is required in patients with impaired renal function.
Concurrent use with aminoglycosides or potent diuretics such as furosemide may increase the risk of nephrotoxicity. Renal function should be monitored closely.

Contraindicated in patients with known hypersensitivity to cefepime, other cephalosporins, penicillins, or beta-lactam antibiotics.
Possible adverse effects include gastrointestinal disturbances, rash, injection site reactions, and hypersensitivity. Serious neurological reactions such as encephalopathy, seizures, and myoclonus may occur, especially in patients with renal impairment receiving unadjusted doses.
Pregnancy Category B. Cefepime should be used during pregnancy only if clearly needed. It is excreted in breast milk in very low concentrations; caution is advised during breastfeeding.
Store as directed. Do not use after the expiry date. Keep out of reach of children. Prescription-only medicine.
