Amlodipine Besilate inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This results in relaxation of peripheral arterioles, reduction of peripheral vascular resistance, and a gradual decrease in blood pressure without significant reflex tachycardia.
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor located in vascular smooth muscle and adrenal glands. Angiotensin II normally causes vasoconstriction and aldosterone release, leading to sodium and water retention. By inhibiting these actions, telmisartan promotes vasodilation and reduces blood volume and blood pressure.
The combination of amlodipine and telmisartan provides an additive and complementary antihypertensive effect, achieving better blood pressure control than either agent alone.

Telmacal Tablet 5 mg + 80 mg is indicated for:
Treatment of essential hypertension in adults
Patients whose blood pressure is not adequately controlled with amlodipine or telmisartan monotherapy
Patients requiring high-dose ARB in combination with a calcium channel blocker
Dosage should be individualized according to patient response and clinical condition
The usual recommended dose is one tablet once daily
Tablets may be taken with or without food
Blood pressure should be monitored regularly during treatment
Dose adjustment may be necessary in elderly patients or those with hepatic impairment
Hypersensitivity to amlodipine, telmisartan, or any component of the formulation
Pregnancy and lactation
Severe hepatic impairment or biliary obstructive disorders
Hypotension may occur, particularly in patients with volume or sodium depletion
Peripheral edema may occur due to amlodipine
Use with caution in patients with renal or hepatic impairment
Avoid concomitant use with other agents acting on the renin-angiotensin system unless clinically indicated
Common side effects may include:
Dizziness or headache
Peripheral edema
Flushing
Fatigue
Hypotension
Most adverse reactions are mild to moderate and tend to resolve with continued therapy or dose adjustment.
Other antihypertensive drugs may enhance the blood pressure-lowering effect
NSAIDs may reduce antihypertensive efficacy and affect renal function
Potassium supplements or potassium-sparing diuretics should be used cautiously
Telmacal Tablet is contraindicated during pregnancy due to the risk of fetal harm
Breastfeeding should be discontinued during treatment
Store below 30°C in a cool and dry place
