Sparlin 200 mg is a potent, broad-spectrum fluoroquinolone antibiotic used to treat a variety of bacterial infections in adults. The active ingredient in Sparlin, a difluoroquinolone, works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication, transcription, repair, and recombination. By interfering with these enzymes, Sparlin prevents bacterial growth and promotes bacterial cell death. This makes it highly effective against Gram-positive, Gram-negative, and atypical bacteria, including Streptococcus pneumoniae, Staphylococcus aureus, Haemophilus influenzae, Mycoplasma pneumoniae, Klebsiella pneumoniae, and Chlamydia pneumoniae.

Sparlin has a long plasma half-life (~16 hours), allowing convenient once-daily dosing in most patients. It also demonstrates excellent tissue penetration, particularly into respiratory tissues, bronchial mucosa, alveolar macrophages, and pleural fluid, ensuring effective treatment of lower respiratory tract infections.

Indications

Sparlin 200 mg is indicated for the treatment of adults (≥18 years) with infections caused by susceptible bacteria, including:

  • Community-acquired pneumonia (CAP)

  • Acute bacterial exacerbations of chronic bronchitis (ABECB)

  • Chronic Obstructive Pulmonary Disease (COPD) exacerbations

  • Acute maxillary sinusitis (AMS)

  • Urinary tract infections, including gonococcal and non-gonococcal urethritis

  • Bacterial prostatitis

  • Bacterial diarrhea

  • Osteomyelitis

  • Other infections including tuberculosis (in combination with rifampicin and isoniazid) and leprosy (in combination with standard drugs)


Therapeutic Class

Pharmacology

Sparlin 200 mg is a synthetic antibacterial agent in the difluoroquinolone family. It binds to bacterial DNA gyrase and topoisomerase IV, inhibiting their supercoiling and replication functions. This results in breakage of bacterial DNA and cell death. In vitro studies show Sparlin to be more active than ciprofloxacin against some Gram-positive organisms and Mycobacteria.

Pharmacokinetics:

  • Absorption: Oral bioavailability ~92%, unaffected by food or milk

  • Distribution: Extensive tissue penetration, particularly in lungs, bronchial mucosa, and alveolar macrophages

  • Metabolism: Mainly hepatic via phase II glucuronidation

  • Excretion: Approximately 50% via urine and 50% via feces, with 10% excreted unchanged in urine


Dosage and Administration

  • Normal renal function: 400 mg on day 1 (two 200 mg tablets), then 200 mg once daily for 10 days

  • Renal impairment (creatinine clearance <50 mL/min): 400 mg on day 1, then 200 mg every 48 hours for 9 days

  • Can be taken with or without food


Contraindications

  • Hypersensitivity to Sparlin or other fluoroquinolones

  • Pregnancy and lactation

  • G6PD deficiency

  • History of Achilles tendinitis with fluoroquinolone use

Precautions and Warnings

  • Phototoxicity: Avoid sunlight; discontinue at early signs of reaction

  • QTc prolongation: Avoid concomitant drugs that increase QT interval

  • Hydration: Ensure adequate hydration to prevent concentrated urine

  • Use caution in renal insufficiency, elderly, and those with pre-existing cardiac or hepatic conditions

  • Safety in children, adolescents (<18 years), pregnant, and lactating women is not established


Pregnancy and Lactation

  • No adequate studies in humans; use only if potential benefit outweighs risk

  • Unknown if excreted in human milk


Special Populations

  • Elderly: No significant pharmacokinetic differences, but monitor renal function

  • Renal impairment: Prolonged half-life; dose adjustment required

  • Hepatic impairment: Mild/moderate impairment does not alter pharmacokinetics
