Primace 5 mg Tablet is indicated for:
Hypertension: Effective in lowering elevated blood pressure, either alone or in combination with other antihypertensive agents
Heart failure: Improves symptoms and survival in patients with chronic heart failure
Post-myocardial infarction: Reduces the risk of mortality and cardiovascular complications in patients recovering from heart attacks
Cardiovascular event prevention: In high-risk patients with conditions like diabetes, coronary artery disease, or a history of stroke
Diabetic nephropathy and chronic kidney disease: Slows progression of kidney damage in patients with proteinuric kidney disease or type 2 diabetes
Ramipril is a prodrug that is metabolized in the liver to ramiprilat, its active form. Ramiprilat inhibits angiotensin-converting enzyme (ACE), preventing the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor. This leads to:
Vasodilation and reduced peripheral resistance
Decreased aldosterone secretion, lowering sodium and water retention
Reduced cardiac workload and improved tissue perfusion
The combination of blood pressure reduction and organ protection makes Ramipril particularly effective in patients with cardiovascular or renal risk factors.
Adults with hypertension: Typically start at 2.5–5 mg once daily, titrated according to response
Heart failure: Initial dose 1.25–2.5 mg once daily, gradually increased as tolerated
Post-myocardial infarction: Started under careful monitoring, usually 2.5 mg twice daily or as directed
Elderly or renal impairment: Begin with lower doses (1.25–2.5 mg once daily) to prevent hypotension
Primace tablets should be taken orally, with or without food, preferably at the same time each day. Swallow the tablet whole with water.
Diuretics: May enhance the hypotensive effect
Potassium supplements or potassium-sparing diuretics: May increase the risk of hyperkalemia
NSAIDs: Can reduce antihypertensive effect and affect kidney function
Lithium: ACE inhibitors may elevate lithium levels, increasing risk of toxicity
Hypersensitivity to Ramipril or other ACE inhibitors
History of angioedema associated with previous ACE inhibitor therapy
Pregnancy, especially in the second and third trimesters
Severe renal impairment or bilateral renal artery stenosis
Common side effects include dizziness, headache, persistent dry cough, hypotension, and fatigue. Rare but serious effects may include angioedema, hyperkalemia, and renal dysfunction.
Monitor blood pressure, renal function, and serum electrolytes regularly
Exercise caution in elderly patients, those with impaired kidney function, or patients receiving multiple antihypertensive medications
Discontinue immediately if signs of angioedema occur
Ramipril is contraindicated during pregnancy due to fetal toxicity. Its use in breastfeeding is not well established; alternative therapy should be considered.
