Primace 1.25 mg Capsule contains Ramipril, an oral antihypertensive agent belonging to the class of ACE inhibitors (Angiotensin-Converting Enzyme inhibitors). It is widely used for the management of high blood pressure (hypertension), heart failure, and for reducing cardiovascular risk in high-risk patients. Primace 1.25 mg Capsule is indicated for:
Hypertension: Alone or in combination with other antihypertensive agents to control high blood pressure
Heart failure: To improve survival and reduce hospitalizations in patients with symptomatic heart failure
Post-myocardial infarction: To reduce cardiovascular morbidity and mortality
Prevention of cardiovascular events: In patients at high risk of myocardial infarction, stroke, or heart failure
Diabetic nephropathy and chronic kidney disease: To slow progression of kidney damage in patients with type 2 diabetes or proteinuric renal disease
Ramipril is a prodrug that is converted in the liver to its active form, ramiprilat. Ramiprilat inhibits the angiotensin-converting enzyme (ACE), preventing the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor. This results in vasodilation, reduced blood pressure, decreased aldosterone secretion, and decreased sodium and water retention. The overall effect is reduced cardiac workload and improved organ perfusion.
Adults with hypertension: Initial dose is usually 2.5 mg once daily; may be titrated according to response.
Elderly or patients with renal impairment: Lower starting dose (1.25 mg once daily) is recommended to reduce the risk of hypotension.
Heart failure: Initial dose typically 1.25–2.5 mg once daily; gradually increased as tolerated.
Post-myocardial infarction: Therapy is started as early as 24 hours post-event with careful monitoring of blood pressure and renal function.
Primace should be taken orally, with or without food, preferably at the same time each day. Capsules should be swallowed whole with water.
Diuretics: Concomitant use may enhance hypotensive effects
Potassium-sparing diuretics, potassium supplements, or salt substitutes: May increase the risk of hyperkalemia
NSAIDs: May reduce the antihypertensive effect and increase risk of kidney dysfunction
Lithium: ACE inhibitors may increase lithium levels and risk of toxicity
Known hypersensitivity to Ramipril or other ACE inhibitors
History of angioedema related to previous ACE inhibitor therapy
Pregnancy (particularly in the second and third trimesters)
Severe renal impairment or bilateral renal artery stenosis
Common side effects of Primace include dizziness, hypotension, headache, fatigue, cough, and gastrointestinal discomfort. Rare but serious reactions may include angioedema, hyperkalemia, and renal impairment.

Blood pressure, renal function, and electrolytes should be monitored regularly
Use caution in patients with renal impairment, elderly patients, or those on concomitant antihypertensive therapy
Discontinue immediately if signs of angioedema appear
Ramipril is contraindicated in pregnancy due to risk of fetal toxicity. Its safety during breastfeeding is not well established; alternative therapy should be considered.
Store capsules in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.
