Prasuva 5 mg Tablet contains Prasugrel, a potent antiplatelet agent belonging to the therapeutic class of anti-platelet drugs. It is primarily used to reduce the risk of thrombotic cardiovascular events in patients with acute coronary syndrome (ACS) who are managed with percutaneous coronary intervention (PCI). By effectively inhibiting platelet activation and aggregation, Prasuva plays a crucial role in preventing serious complications such as myocardial infarction, stroke, and stent thrombosis.

Indications

Prasuva 5 mg is indicated to reduce the rate of thrombotic cardiovascular (CV) events, including stent thrombosis, in patients with acute coronary syndrome who are undergoing PCI, including:

  • Patients with unstable angina (UA)

  • Patients with non-ST-elevation myocardial infarction (NSTEMI)

  • Patients with ST-elevation myocardial infarction (STEMI) managed with primary or delayed PCI

Pharmacology and Mode of Action

Prasugrel is a prodrug that is rapidly converted to its active metabolite following oral administration. The active metabolite irreversibly binds to the P2Y12 adenosine diphosphate (ADP) receptor on platelets. This action inhibits platelet activation and aggregation for the lifespan of the platelet, thereby reducing the formation of thrombotic events.

Dosage and Administration

Treatment should be initiated with a single 60 mg oral loading dose, followed by a maintenance dose of 10 mg once daily, with or without food.

  • A reduced maintenance dose of 5 mg once daily is recommended for patients weighing less than 60 kg.

  • Prasuva should be administered in combination with aspirin (75–325 mg daily) unless contraindicated.

Drug Interactions

Concomitant use of Prasuva with warfarin or chronic use of NSAIDs may increase the risk of bleeding. Prasugrel can be safely coadministered with aspirin, heparin, GPIIb/IIIa inhibitors, statins, proton pump inhibitors, H2 blockers, and drugs that induce or inhibit cytochrome P450 enzymes.

Contraindications

Prasuva is contraindicated in patients with:

  • Active pathological bleeding, such as peptic ulcer disease or intracranial hemorrhage

  • A history of transient ischemic attack (TIA) or stroke

Side Effects

The most common adverse effect associated with Prasuva is bleeding. Other reported side effects include thrombotic thrombocytopenic purpura, headache, back pain, dyspnea, nausea, hypertension, bradycardia, and skin rash.

Precautions and Warnings

Use in Special Populations

  • Geriatric patients: Patients over 75 years of age have an increased risk of fatal bleeding; use with caution.

  • Renal impairment: No dosage adjustment is required; limited data are available in end-stage renal disease.

  • Hepatic impairment: No dosage adjustment is required in mild to moderate hepatic impairment.

  • Pediatric use: Safety and effectiveness have not been established.

Pregnancy and Lactation

There are no adequate and well-controlled studies of Prasugrel use in pregnant women. Animal studies have shown no evidence of fetal harm; however, Prasuva should be used during pregnancy only if clearly needed. It is not known whether Prasugrel is excreted in human milk; caution is advised when administered to nursing mothers.
