Pacet 100 mg Tablet contains Amiodarone, a broad-spectrum antiarrhythmic medication widely used for the management of serious and life-threatening cardiac rhythm disorders. Indications

Pacet 100 mg is indicated for the treatment and prevention of various cardiac arrhythmias, including:

• Ventricular fibrillation

• Ventricular tachycardia

• Atrial fibrillation

• Atrial flutter

Amiodarone injection is additionally indicated for the initiation of treatment and prophylaxis of frequently recurring ventricular fibrillation (VF) and hemodynamically unstable ventricular tachycardia (VT) in patients who are refractory to other therapies.

Pharmacology

Amiodarone exerts its antiarrhythmic effects through multiple mechanisms. It prolongs the myocardial cell action potential duration and refractory period across all cardiac tissues. The drug also demonstrates non-competitive antagonism of both alpha and beta adrenergic receptors. These actions lead to a reduction in abnormal electrical activity, decreased automaticity, and stabilization of cardiac rhythm.

Electrophysiological effects include a reduction in sinus rate, prolongation of PR and QT intervals, development of U-waves, and changes in T-wave contour. While these changes reflect the pharmacological action of Amiodarone, careful monitoring is required due to the potential risk of bradycardia and conduction disturbances.

Dosage and Administration

Tablet:
The usual oral loading dose is 200 mg three times daily for one week, followed by 200 mg twice daily or the minimum effective dose required to control arrhythmia. Tablets should be taken with meals to reduce gastrointestinal discomfort.

Injection:
The recommended initial dose is approximately 1000 mg over the first 24 hours, administered as a controlled intravenous infusion according to standard protocols.

Drug Interactions

Pacet 100 mg may interact with beta-blockers, calcium channel blockers, digoxin, warfarin, flecainide, procainamide, quinidine, tricyclic antidepressants, and dextromethorphan. These interactions may increase the risk of bradycardia, bleeding, or elevated drug concentrations, requiring close monitoring and dose adjustments.

Contraindications

Amiodarone is contraindicated in patients with cardiogenic shock, severe sinus node dysfunction, second or third-degree atrioventricular block without a pacemaker, iodine hypersensitivity, and in cases where bradycardia has caused syncope.

Side Effects

Serious adverse effects may involve the lungs, liver, thyroid, eyes, skin, and nervous system. Commonly reported effects include bradycardia, photosensitivity, thyroid dysfunction, corneal microdeposits, gastrointestinal disturbances, and elevated liver enzymes. Severe reactions may require dose reduction or discontinuation.

Pregnancy and Lactation

Pacet 100 mg should not be used during pregnancy or lactation due to potential risks to the fetus and infant.
