Monocast 5 mg Tablet is a leukotriene receptor antagonist used in the long-term management of asthma and allergic conditions. It contains Montelukast 5 mg, which helps control inflammation in the airways and reduces allergy-related symptoms. Monocast is not intended for immediate relief of acute asthma attacks but is highly effective as a preventive and maintenance therapy in both pediatric and adult patients.

This medication plays an important role in improving respiratory function, reducing asthma exacerbations, and providing relief from symptoms of seasonal and perennial allergic rhinitis.

Indications

Monocast 5 mg is indicated for:

  • Prophylaxis and chronic treatment of asthma

  • Prevention of exercise-induced bronchoconstriction (EIB)

  • Relief of symptoms associated with allergic rhinitis, including:

    • Seasonal allergic rhinitis

    • Perennial allergic rhinitis


Therapeutic Class

  • Leukotriene Receptor Antagonists

  • Antibodies to leukotriene receptors


Pharmacology and Mode of Action

Asthma and allergic rhinitis are associated with the release of inflammatory mediators known as cysteinyl leukotrienes (LTC₄, LTD₄, LTE₄) from mast cells and eosinophils. These mediators bind to leukotriene receptors (CysLT₁) located in the human airway, causing bronchoconstriction, airway edema, and increased mucus secretion.

Monocast 5 mg selectively and strongly blocks the CysLT₁ receptor, inhibiting the action of leukotriene D₄. By preventing leukotriene-mediated inflammation without any agonist activity, Monocast helps reduce airway narrowing, improve breathing, and alleviate allergy symptoms.


Dosage and Administration

Adults and Adolescents (≥15 years):

  • Montelukast 10 mg tablet once daily

Pediatric Patients:

  • 6 to 14 years: Monocast 5 mg tablet once daily

  • 2 to 5 years: Montelukast 4 mg tablet once daily

  • 6 months to 5 years: Montelukast 4 mg oral granules once daily

Granules may be administered directly into the mouth or mixed with a small amount of cold water or soft food at room temperature.

Special Populations:

  • Hepatic insufficiency (mild to moderate): No dose adjustment required

  • Renal insufficiency: No dose adjustment required

  • Elderly patients: No dose adjustment necessary


Drug Interactions

Monocast 5 mg can be safely used with many commonly prescribed medications, including inhaled corticosteroids, bronchodilators, antihistamines, and oral contraceptives. Clinically significant interactions have not been observed with theophylline, warfarin, digoxin, prednisone, or NSAIDs.

Strong hepatic enzyme inducers such as phenobarbital or rifampicin may reduce plasma concentrations of Montelukast; however, routine dose adjustment is not generally required. Clinical monitoring is advised.


Contraindications

  • Known hypersensitivity to Montelukast or any component of the formulation


Side Effects

Common:
Upper respiratory tract infection, headache, fever, diarrhea, nausea, vomiting, abdominal pain

Uncommon:
Dizziness, drowsiness, anxiety, irritability, sleep disturbances, muscle or joint pain

Rare:
Pulmonary eosinophilia, angioedema, liver dysfunction, hallucinations, depression, suicidal thoughts, tremor

Patients should report any unusual behavioral or mood changes to a healthcare provider.

Pregnancy and Lactation

Monocast 5 mg is classified as Pregnancy Category B. Animal studies show placental transfer, but adequate studies in pregnant women are lacking. Use during pregnancy and breastfeeding should be based on clinical necessity and medical advice.


Precautions and Warnings

  • Not indicated for the treatment of acute asthma attacks or status asthmaticus

  • Patients should continue using rescue inhalers as prescribed

  • Inhaled or oral corticosteroids should not be stopped abruptly

  • Patients with aspirin sensitivity should continue avoiding aspirin and NSAIDs


Storage Conditions

Store below 30°C in a cool, dry place, protected from light and moisture. Keep out of reach of children.
