Adults:

Heart Failure: Start 2.5–5 mg/day, titrate by ≤10 mg every 2 weeks, max 40 mg/day.
Acute MI: Start 5 mg/day within 24 hours, increase to 10 mg/day.
Children (≥6 years):
Hypertension: 0.07 mg/kg/day, up to 5 mg once daily
Can be taken with or without food.
Enhances hypotensive effect with diuretics
NSAIDs may reduce antihypertensive effect and worsen renal function
Lithium: Increased risk of toxicity
Potassium supplements or K-sparing diuretics: Increased risk of hyperkalemia
Gold sodium thiomalate: Increased risk of nitritoid reactions
History of angioedema related to ACE inhibitors
Concomitant use with aliskiren in patients with diabetes or renal impairment
Pregnancy (Category D)
Children with GFR <30 mL/min/1.73 m²
Common side effects include:
Headache, fatigue, dizziness
Persistent non-productive cough
Chest or abdominal pain, nausea, vomiting, diarrhea
Rash, hypotension, asthenia
Renal dysfunction, hyperkalemia, increased BUN/creatinine
Rare: intestinal angioedema
Bilateral renal artery stenosis or single kidney with stenosis
Collagen vascular disease
Risk of angioedema higher in black patients
Caution in lactation
Avoid in children with severe renal impairment
Symptoms: Hypotension, tachycardia/bradycardia, shock, dizziness, electrolyte imbalance
Treatment: Normal saline infusion, gastric lavage, activated charcoal, dialysis if necessary, IV catecholamines or angiotensin II infusion in severe cases
