Sitagliptin works by inhibiting the DPP-4 enzyme, which is responsible for breaking down incretin hormones such as GLP-1 (Glucagon-Like Peptide-1) and GIP (Glucose-Dependent Insulinotropic Polypeptide). By blocking DPP-4:
Levels of active incretin hormones increase
Insulin secretion from pancreatic beta cells is enhanced in a glucose-dependent manner
Glucagon secretion from alpha cells is reduced
Hepatic glucose production decreases
This dual effect lowers both fasting and postprandial blood glucose levels. Because its action depends on elevated blood glucose, the risk of severe hypoglycemia is relatively low compared to some other antidiabetic medications.
Janvia Tablet 100 mg is indicated for:
Management of Type 2 Diabetes Mellitus in adults
Monotherapy when diet and exercise are inadequate
Combination therapy with Metformin, sulfonylureas, thiazolidinediones, or insulin
Patients requiring effective glycemic control with weight-neutral therapy
It is not indicated for Type 1 diabetes or diabetic ketoacidosis.
The usual recommended dose is 100 mg once daily, taken with or without food. Dose adjustment is necessary in patients with moderate to severe renal impairment. The tablet should be swallowed whole with water.
Regular monitoring of blood glucose levels and HbA1c is recommended to evaluate treatment effectiveness and guide therapy adjustments.
