Inclox 500 mg contains Flucloxacillin, a penicillinase-resistant penicillin antibiotic used for the treatment of infections caused by Gram-positive bacteria, particularly penicillinase-producing staphylococci. It is a bactericidal antibiotic widely prescribed for skin, respiratory, bone, and systemic infections where resistant staphylococcal strains are suspected or confirmed. Inclox 500 mg is also used prophylactically during certain major surgical procedures to prevent postoperative infections.

Penicillinase-resistant penicillins
Flucloxacillin is an isoxazolyl penicillin that inhibits bacterial cell wall synthesis, leading to bacterial cell death. It acts by binding to penicillin-binding proteins (PBPs) and blocking the transpeptidase enzyme involved in the final stage of peptidoglycan synthesis. This prevents cross-linking of the bacterial cell wall, resulting in weakening, swelling, and eventual rupture of the bacterial cell.
Flucloxacillin is highly resistant to hydrolysis by staphylococcal penicillinase due to steric hindrance from its acyl side chain. It is stable in acidic conditions and is particularly effective against Gram-positive organisms, while its activity against Gram-negative bacilli is limited.
Inclox 500 mg is indicated for the treatment of infections caused by susceptible Gram-positive organisms, including penicillinase-producing staphylococci, such as:
Skin and Soft Tissue Infections
Boils, abscesses, carbuncles
Infected ulcers, eczema, acne, furunculosis
Cellulitis, infected wounds, and burns
Respiratory Tract Infections
Pneumonia
Lung abscess
Empyema
Sinusitis, pharyngitis, tonsillitis, quinsy
Other Infections
Osteomyelitis
Enteritis
Endocarditis
Urinary tract infections
Meningitis
Septicaemia
Prophylactic Use
Prevention of infections during major surgical procedures, especially cardiothoracic and orthopedic surgeries, when clinically indicated
Oral Administration
Adults: 250 mg four times daily; dose may be doubled in severe infections
Osteomyelitis and endocarditis: Up to 8 g daily in divided doses every 6–8 hours
Children (2–10 years): Half of the adult dose
Children (under 2 years): One quarter of the adult dose
Parenteral Administration
Adults/Elderly:
Intramuscular: 250 mg four times daily
Intravenous: 250 mg to 1 g four times daily by slow IV injection or infusion
Severe infections: Doses may be doubled; up to 8 g daily for endocarditis or osteomyelitis
Children:
2–10 years: Half of the adult dose
Under 2 years: One quarter of the adult dose
Administration Note:
Oral doses should be taken 1 hour before meals for optimal absorption.
In patients with severe renal failure (creatinine clearance <10 ml/min), dose reduction or prolongation of dosing interval should be considered.
Inclox 500 mg is contraindicated in patients with known hypersensitivity to penicillins.

Common adverse effects include nausea, vomiting, diarrhea, dyspepsia, and other mild gastrointestinal disturbances. Hypersensitivity reactions such as rash, urticaria, fever, purpura, interstitial nephritis, hepatitis, and cholestatic jaundice have been reported.
Caution is advised in patients with existing liver dysfunction or a history of allergic reactions. Liver function should be monitored during prolonged therapy.
Renal impairment: Dose reduction or extended dosing interval is recommended in severe renal failure
Pediatric patients: Dosage adjustment is required based on age and renal function
Inclox 500 mg is classified as US FDA Pregnancy Category B. There are no adequate, well-controlled studies in pregnant women; therefore, it should be used during pregnancy only if clearly necessary. Flucloxacillin is excreted in breast milk, and caution should be exercised when administered to lactating mothers.
Store in a dry place away from light and heat. Keep out of reach of children.
