Indications

Imacent Tablet 100 mg is indicated for the treatment of:

• Philadelphia chromosome–positive (Ph+) chronic myeloid leukemia (CML) in chronic phase, accelerated phase, or blast crisis

• Philadelphia chromosome–positive acute lymphoblastic leukemia (Ph+ ALL)

• Gastrointestinal stromal tumors (GIST) that are unresectable, metastatic, or recurrent

• Myelodysplastic/myeloproliferative diseases associated with specific genetic abnormalities

• Hypereosinophilic syndrome and chronic eosinophilic leukemia

• Dermatofibrosarcoma protuberans (DFSP)

Pharmacology / Mode of Action

Imatinib Mesylate inhibits multiple tyrosine kinases, including BCR-ABL, c-KIT, and platelet-derived growth factor receptors (PDGFR). In CML, the BCR-ABL fusion protein produced by the Philadelphia chromosome drives malignant cell proliferation. Imatinib blocks this abnormal kinase activity, suppressing leukemic cell growth and promoting apoptosis. In GIST, inhibition of c-KIT signaling leads to reduced tumor cell survival. This targeted mechanism allows selective action on cancer cells while sparing normal tissues.

Dosage and Administration

The dosage of Imacent Tablet 100 mg depends on the type of cancer, disease phase, and patient response.

• CML (chronic phase): Total daily dose of 400 mg, administered as four 100 mg tablets once daily or in divided doses

• Advanced CML or Ph+ ALL: Higher doses may be prescribed under specialist supervision

• GIST: Typically 400 mg daily, adjusted based on response and tolerance

Imacent Tablet should be taken orally with food and a full glass of water to minimize gastrointestinal discomfort. Tablets should be swallowed whole and not crushed or chewed unless directed by a physician.

Contraindications

Imacent Tablet 100 mg is contraindicated in patients with known hypersensitivity to Imatinib Mesylate or any of its components.

Side Effects

Common adverse effects include nausea, vomiting, diarrhea, abdominal pain, edema, muscle cramps, fatigue, headache, skin rash, and weight gain. Hematological abnormalities such as neutropenia, anemia, and thrombocytopenia may occur, particularly during the initial phase of treatment. Drug Interactions

Imatinib is metabolized by the CYP3A4 enzyme system. Concomitant use with strong CYP3A4 inhibitors or inducers may affect drug levels. Caution is advised when used with warfarin, paracetamol, antifungal agents, and certain antiepileptic drugs.

Pregnancy and Lactation

Imacent Tablet should not be used during pregnancy unless clearly necessary, as it may pose a risk to the fetus. Effective contraception is advised for women of reproductive potential. Breastfeeding should be discontinued during treatment due to potential excretion in breast milk.

Precautions and Warnings
