Flamyd 500 mg is a broad-spectrum antiprotozoal and antibacterial agent from the imidazole class. It is primarily indicated for the treatment and prevention of infections caused by anaerobic bacteria and protozoa. Its specific uses include:
Prevention of post-operative infections caused by anaerobic bacteria, particularly Bacteroides species and anaerobic streptococci.
Treatment of serious anaerobic infections, including:
Septicaemia and bacteraemia
Peritonitis
Brain abscess
Pelvic abscess and pelvic cellulitis
Post-operative wound infections
Urogenital infections:
Trichomoniasis
Bacterial vaginosis (non-specific vaginitis)
Protozoal infections:
Amoebiasis (intestinal, extra-intestinal, and asymptomatic cyst carriers)
Giardiasis
Oral and dental infections:
Acute ulcerative gingivitis
Acute dental infections due to anaerobic organisms
Skin and soft tissue infections:
Anaerobically infected leg ulcers and pressure sores
Special conditions:
Antibiotic-associated pseudomembranous colitis
Surgical prophylaxis to prevent anaerobic infections in high-risk surgeries

Flamyd 500 mg is metabolically reduced by anaerobic organisms. Its active reduced form interacts with microbial DNA, disrupting nucleic acid synthesis and causing bactericidal and protozoacidal effects. This mechanism makes it effective against anaerobic bacterial infections and protozoal diseases such as amoebiasis and giardiasis.
Tablets and Suspensions: Dose varies by age, condition, and severity. For adults, common doses range from 200 mg three times daily (tid) to 2 g as a single dose depending on the infection.
Vaginal Gel: One applicator (~5 g) once or twice daily for 5 days.
Suppository: 1 g every 8–12 hours depending on the infection; pediatric doses are weight-based.
IV Infusion: Adults and children >12 years: 500 mg every 8 hours at 5 ml/min; max 4 g/24h. Children <12 years: 7.5 mg/kg every 8 hours.
Disulfiram & Alcohol: Can cause a disulfiram-like reaction (flushing, vomiting, tachycardia).
Warfarin: May potentiate anticoagulant effect; monitor prothrombin time.
Lithium & Cyclosporin: May increase serum levels, requiring monitoring.
Phenytoin/Phenobarbital: May reduce plasma levels of Flamyd.
5-Fluorouracil & Busulfan: Clearance may be affected, increasing toxicity risk.
Hypersensitivity to metronidazole or other nitroimidazoles.
Metallic taste, nausea, vomiting, diarrhea, drowsiness, and rashes.
Monitor blood counts with prolonged use.
Caution in hepatic encephalopathy; may require dose reduction.
Urine may darken during treatment.
FDA Pregnancy Category B; use only if clearly needed.
Excreted in human milk; exercise caution in nursing mothers.
Pediatric doses are weight-based.
Renal impairment: no dose adjustment needed.
Hepatic impairment: use caution and reduce dose in severe liver disease.
