Indications

Fenofex 120 mg is indicated for:

• Allergic rhinitis (seasonal and persistent) in adults and children 12 years and older

  • Sneezing

  • Rhinorrhea

  • Lacrimation

  • Itchy, red eyes

  • Itching of the nose, palate, and throat

• Chronic idiopathic urticaria in adults and children 12 years and older

  • Reduction in wheals

  • Relief from pruritus

Pharmacology

Fexofenadine is a selective peripheral H1-receptor antagonist. After oral administration, it is rapidly absorbed and reaches peak plasma concentration within 2 to 3 hours. Unlike first-generation antihistamines, fexofenadine does not readily cross the blood-brain barrier, which explains its minimal sedative effect and lack of significant central nervous system involvement.

Mode of Action

Allergic reactions are mediated by histamine released from mast cells and basophils following allergen exposure. Histamine activates H1 receptors, leading to symptoms such as itching, rhinorrhea, and watery eyes. Fexofenadine acts as an inverse agonist at the H1 receptor, stabilizing the receptor in its inactive form and preventing downstream inflammatory effects. It has no significant anticholinergic, antidopaminergic, or sedative activity.

Dosage & Administration

Allergic Rhinitis

• Adults and children ≥12 years:

  • 60 mg twice daily or

  • 120 mg once daily or

  • 180 mg once daily

• Renal impairment: 60 mg once daily

Chronic Idiopathic Urticaria

• Adults and children ≥12 years:

  • 60 mg twice daily or

  • 120 mg once daily or

  • 180 mg once daily

• Renal impairment: 60 mg once daily

Dosage adjustments are recommended for children and patients with impaired renal function.

Drug Interactions

Fenofex has a low potential for drug interactions as it does not undergo hepatic metabolism. Co-administration with erythromycin or ketoconazole may increase plasma fexofenadine levels without clinical significance. Antacids containing aluminum or magnesium hydroxide may reduce absorption; a 2-hour interval between dosing is recommended.

Contraindications

Fenofex 120 mg is contraindicated in patients with known hypersensitivity to fexofenadine or any of its components.

Side Effects

Fenofex is generally well tolerated. Commonly reported side effects include:

• Headache

• Fatigue

• Dizziness

• Nausea

Rare post-marketing effects include hypersensitivity reactions, palpitations, insomnia, rash, and pruritus.

Pregnancy & Lactation

Fenofex 120 mg is classified as Pregnancy Category C. Its use during pregnancy and breastfeeding is not recommended unless clearly necessary and prescribed by a physician.

Precautions & Warnings

Caution is advised in elderly patients and those with renal or cardiovascular disease. Fenofex is unlikely to impair driving or operating machinery, but individual response should be assessed.
