Congestive heart failure following an acute myocardial infarction

Pharmacology / Mode of Action:
Epleron 50 mg binds selectively to mineralocorticoid receptors in both epithelial tissues (kidneys, gastrointestinal tract) and nonepithelial tissues (heart, blood vessels, brain). By blocking aldosterone, it reduces sodium and water retention, decreases blood pressure, and prevents vascular and cardiac remodeling, thereby supporting cardiovascular function.
Dosage & Administration:
Congestive Heart Failure after Myocardial Infarction:
Start at 25 mg once daily and increase to 50 mg once daily within 4 weeks as tolerated. Can be taken with or without food.
Hypertension:
Recommended starting dose is 50 mg once daily, either alone or with other antihypertensives. If necessary, increase to 50 mg twice daily. Do not exceed 100 mg daily due to risk of hyperkalemia.
Interactions:
May increase hyperkalemia risk with ACE inhibitors, ARBs, ciclosporin, tacrolimus, or trimethoprim.
NSAIDs, glucocorticoids, and tetracosactide may reduce antihypertensive effects.
May enhance hypotensive effect with α1-blockers, TCAs, amifostine, baclofen, or neuroleptics.
Mild to moderate CYP3A4 inhibitors (e.g., fluconazole, erythromycin, diltiazem) may increase plasma levels.
Contraindications:
Hyperkalemia
Severe renal impairment (creatinine clearance <30 mL/min)
Severe hepatic impairment
Concomitant use with potent CYP3A4 inhibitors or other potassium-sparing diuretics
Side Effects:
Common: Headache, dizziness, diarrhea, stomach pain, nausea, cough, or flu-like symptoms
Serious: Rash, swelling, severe dizziness, or difficulty breathing
Pregnancy & Lactation:
Use only if potential benefits outweigh risks during pregnancy.
Unknown excretion in breast milk; assess whether to discontinue drug or breastfeeding.
Precautions & Warnings:
Use caution in patients with hyperkalemia, severe kidney disease, or diabetes with heart failure after myocardial infarction.
Overdose Management:
Symptoms: Hyperkalemia, hypotension
Treatment: Symptomatic and supportive care, including activated charcoal and standard therapy for hyperkalemia
Storage Conditions:
Store below 30°C in a cool, dry place, protected from light and moisture. Keep out of reach of children.
Special Populations:
Pediatric: Safety and efficacy not established
Geriatric: No significant differences observed in effectiveness or safety
