Adafil 20 mg is indicated for:
Erectile Dysfunction (ED)
Benign Prostatic Hyperplasia (BPH)
Treatment of patients with both Erectile Dysfunction and signs and symptoms of Benign Prostatic Hyperplasia
Adafil 20 mg acts as a selective phosphodiesterase type 5 (PDE5) inhibitor. PDE5 is responsible for the degradation of cyclic guanosine monophosphate (cGMP). By inhibiting this enzyme, tadalafil increases cGMP levels within smooth muscle cells. Elevated cGMP leads to smooth muscle relaxation and enhanced blood flow to the corpus cavernosum, resulting in a natural erectile response during sexual stimulation.
PDE5 is also found in the smooth muscles of the prostate and bladder wall. Inhibition of PDE5 in these tissues leads to relaxation of smooth muscle, improved blood flow in the lower urinary tract, and widening of the bladder neck. These effects help reduce urinary symptoms such as difficulty in urination, weak stream, and incomplete bladder emptying associated with BPH.
A distinguishing feature of Adafil 20 mg is its long duration of action, remaining effective for up to 36 hours, allowing greater flexibility in sexual activity.
Erectile Dysfunction:
The recommended starting dose is 10 mg taken prior to anticipated sexual activity. Based on individual response and tolerability, the dose may be increased to 20 mg or decreased to 5 mg. The maximum recommended dosing frequency is once daily.
Benign Prostatic Hyperplasia:
The recommended dose is 5 mg taken once daily at the same time each day.
Combined ED and BPH:
A dose of 5 mg once daily at the same time each day is recommended.
Adafil 20 mg may be taken with or without food.
Adafil 20 mg may interact with nitrates such as nitroglycerin and isosorbide, alpha-adrenergic blockers, antihypertensive drugs, alcohol, antacids containing magnesium or aluminum hydroxide, and strong CYP3A4 inhibitors including ketoconazole, itraconazole, ritonavir, and erythromycin. Interactions may also occur with grapefruit juice, HIV protease inhibitors, rifampin, carbamazepine, phenytoin, and phenobarbital.
Concomitant use of nitrates due to the risk of severe hypotension
Known hypersensitivity to tadalafil or any component of the formulation
Common side effects include headache, dyspepsia, back pain, myalgia, nasal congestion, and nasopharyngitis. Rare but serious adverse events may include visual disturbances, sudden vision or hearing loss, severe skin reactions, angina, myocardial infarction, stroke, severe hypotension, and tachycardia.

Adafil 20 mg is classified as USFDA pregnancy category B. Use during pregnancy is recommended only if the potential benefit outweighs the risk. There are no adequate data regarding its excretion into human breast milk; caution is advised when administered to nursing women.
Caution should be exercised in patients with cardiovascular disease, angina, renal or hepatic impairment, bleeding disorders, and those using nitrates, alpha blockers, alcohol, CYP3A4 inhibitors, or other PDE5 inhibitors.
Store in a dry place away from light and heat. Keep out of the reach of children.
